The Educational Commission for Foreign Medical Graduates (ECFMG®), is a private nonprofit organization committed to promoting excellence in international medical education. ECFMG, and its Foundation for Advancement of International Medical Education and Research, offer a variety of programs and services for international physicians and the international medical community.
Through its program of certification, ECFMG evaluates the qualifications of graduates of international medical schools who seek entry into residency or fellowship programs in the United States that are accredited by the Accreditation Council for Graduate Medical Education (ACGME). International medical graduates who wish to enter such programs must be certified by ECFMG. The current requirements for ECFMG Certification include passing the Step 1 and 2 examinations of the three-step United States Medical Licensing Examination® (USMLE®).
A clinical skills (CS) component became part of the USMLE Step 2 examination in June 2004. ECFMG, in collaboration with the National Board of Medical Examiners®, administers Step 2 CS at clinical skills evaluation centers in Atlanta, Chicago, Houston, Los Angeles, and Philadelphia. Step 2 CS assesses an examinee's ability to gather and interpret clinical patient data and communicate effectively in spoken and written English. The assessment model used for Step 2 CS depends heavily on standardized patients (SPs). The SPs simulate actual patients by presenting symptoms and responding to questions from examinees with answers appropriate to the patient they portray. Graduates of both international medical schools and medical schools in the United States and Canada must take Step 2 CS.
